Arduino A Mangoni is Strategic Professor of Clinical Pharmacology at Flinders University, Senior Consultant in Clinical Pharmacology and General Medicine and Head of the Department of Clinical Pharmacology at Flinders Medical Centre, Southern Adelaide Local Health Network, Adelaide, Australia[1][2]. His academic career has focused on investigating the effects of pharmacological and non-pharmacological interventions on the cardiovascular system, the identification of new age-related disease biomarkers and drugs targeting arginine metabolic pathways, and the safety and quality use of drugs in older patients. His clinical career has been dedicated to the management of patients with hypertension and cardiovascular risk and the implementation of deprescribing and medication management strategies in older patients. Professor Mangoni was awarded a Carrick citation for his outstanding contributions to student learning "For promotion of safe and effective medication management through an innovative e-learning pharmacology course for Nurse Practitioners in urban, rural and remote areas of Australia"[3]. Since 2011, he was awarded 6 Visiting Professorships by the University of Sassari to undertake teaching and research activities in the area of biomarker discovery in age-associated disease states[4][5]. In 2012, he was awarded the Fellowship of the British Pharmacological Society for his outstanding contributions and peer recognition in the field of pharmacology[6]. In 2019, he was awarded a Honorary Professorship in Clinical Pharmacology at the Technische Universität Dresden (Germany)[7], and in 2020 the International Society of Hypertension Fellowship for his outstanding contributions in hypertension research and patient care[8]. He is co-Editor and author of the book "Prescribing for Elderly Patients"[9].

Education and career

Mangoni graduated in Medicine at the University of Milan in 1991. He then completed specialist training in Cardiology, Clinical Pharmacology & Therapeutics, and Internal Medicine in Italy, USA, and the UK. After being awarded a PhD in Pharmacology at King’s College, University of London, in 2003, he was appointed Senior Lecturer (2003-2007) and promoted to Associate Professor (2007-2009) in Clinical Pharmacology at Flinders University, Adelaide, Australia, before taking up the Chair in Medicine of Old Age at the University of Aberdeen (2010-2012). Since 2013, he is Strategic Professor of Clinical Pharmacology at Flinders University. In 2015-2016, he was Associate Dean and Head, Flinders Medical Science and Technology. He is elected Fellow of the Royal College of Physicians of London[10], Royal College of Physicians and Surgeons of Glasgow[11], Royal College of Physicians of Edinburgh[12], and Royal Australasian College of Physicians[13].

Research

Vasoreactivity, arterial function, and hypertension

Mangoni’s early research career focused on investigating the effects of pharmacological and non-pharmacological interventions on vasoreactivity and arterial function. He identified the critical role of heart rate and sympathetic activity in modulating arterial distensibility in animal studies[14][15]. He then demonstrated that folic acid supplementation significantly enhances endothelial-dependent vasodilatation and reduces blood pressure in high cardiovascular risk patient groups, independently of homocysteine lowering[16][17]. More recently, he participated in landmark studies defining ambulatory blood pressure targets for the diagnosis and the treatment of hypertension and the dissemination of relevant professional recommendations[18][19].

Pharmacoepidemiology and drug safety

Mangoni has investigated the cardiovascular safety of non-steroidal anti-inflammatory drugs[20], and the adverse effects of anticholinergic drugs in Australian and European cohorts[21][22][23]. More recently, he has contributed to the validation and adaptation of scoring tools predicting adverse outcomes in older patients using machine learning techniques[24], and the investigation of temporal trends in medication use in Australia[25].

Biomarker and drug discovery

Mangoni has identified, as part of an international research consortium, the biological and clinical significance of biomarkers within the arginine metabolic pathways in Australian and European patient cohorts [26][27][28][29][30]. Furthermore, he has discovered the critical role of small molecule arginine analogue inhibitors of the enzyme dimethylarginine dimethylaminohydrolase-1 in suppressing neovascularization in experimental cancer models[31][32][33].

Professional contributions

Mangoni is Editor-in-Chief of "Therapeutic Advances in Drug Safety"[34], "Current Reviews in Clinical and Experimental Pharmacology"[35], and "Clinical Medicine Insights: Cardiology"[36]. As Executive Member of the National Blood Pressure and Vascular Disease Committee of the National Heart Foundation and the Working Group of the High Blood Pressure and Research Council Australia, he revised and updated the Australian Hypertension Guidelines[37], and the Australian Guidelines on the use of Ambulatory Blood Pressure Monitoring for the management of Hypertension[19]. As Executive Member of the Guideline Development Group, Scottish Intercollegiate Guidelines Network (SIGN), he contributed to the development of the guidelines for the Management of Chronic Pain[38]. As Advisory Panel Member of the Australian College of Emergency Medicine, he contributed to the review of the efficacy and safety of thrombolysis in acute ischaemic stroke[39].
